Release and Waiver of Liability

In consideration for being permitted to participate in any way in Tae Kwon Do or Altitude Fitness and Martial Arts activities, I, the undersigned, understand, acknowledge and accept that:

There is a risk that INJURIES, DAMAGES AND/OR DEATH may result from participating in Tae Kwon Do or Altitude Fitness and Martial Arts activities.

I understand and acknowledge the dangers associated with the consumption of alcohol or any mind altering drugs and agree not to drink alcohol or take drugs prohibited by law before or during any Tae Kwon Do or Altitude Fitness and Martial Arts activities.

I undertake not to sue Altitude Fitness and Martial Arts or their employees or agents for any claims, costs, damages, or other liabilities they may have for injury suffered by me and I acknowledge that this waiver represents a legal release and discharge of legal responsibility to Altitude Fitness and Martial Arts or their employees or agents.

I agree to follow the directions of my instructors and that any misconduct or refusal by me to follow any direction of any instructor can result in the CANCELLATION of my participation in the activities and my immediate removal from the premises NO MATTER where that may occur.

I agree to wear approved uniform and protective equipment at all times whilst participating in the martial art of Tae Kwon Do or Altitude Fitness and Martial Arts activities where this is required under the relevant rules and regulations or as seen fit by instructors.

I have had sufficient opportunity to read this Dangerous Activity Acknowledgement and fully understand its terms and sign it freely and voluntarily.
